Job Summary:
The County of San Mateo, CA is seeking a highly skilled and compassionate Social Worker II/III to join our Aging and Adult Services team. As an In-Home Supportive Services Social Worker, you will play a critical role in providing supportive services to individuals who require assistance with daily living activities.
Key Responsibilities:
- Conduct home visits to assess the needs of clients and develop individualized service plans.
- Provide case management services to ensure clients receive the necessary support and resources.
- Collaborate with healthcare providers, family members, and other stakeholders to ensure comprehensive care.
- Develop and maintain relationships with community resources and service providers to ensure access to necessary services.
- Participate in quality improvement initiatives to enhance the delivery of services.
Requirements:
- Master's degree in Social Work or related field.
- Two years of experience in social work or a related field, with a focus on case management and service coordination.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Fluency in English and one or more of the following languages: Cantonese, Mandarin, Tagalog, Vietnamese, or Farsi.
