As a skilled Home Health Registered Nurse, you will play a vital role in delivering high-quality patient care in the comfort of their own homes. Your expertise will enable patients to recover, rehabilitate, and regain their independence, empowering them to live healthier and happier lives.

Key Responsibilities:
  • Provide comprehensive care and case management for home health patients, ensuring seamless transitions and optimal outcomes.
  • Collaborate with physicians to develop and implement personalized care plans, addressing the unique needs and goals of each patient.
  • Conduct thorough assessments, identify areas for improvement, and implement evidence-based interventions to enhance patient outcomes.
  • Coordinate with interdisciplinary teams, including physical therapists, occupational therapists, and home health aides, to ensure comprehensive care and support.
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and seamless care transitions.
  • Document patient progress, outcomes, and care plans accurately and thoroughly, maintaining compliance with regulatory requirements.
Requirements:
  • Diploma, Associate, or Bachelor's degree in Nursing
  • Minimum one year of nursing experience, with a strong background in med-surg, ICU, ER, or acute care
  • Home health experience a plus, but not required
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Strong organizational, communication, and interpersonal skills
  • Valid driver's license, auto insurance, and reliable transportation
