Responsibilities
The Medical Practitioner Leader will be responsible for managing acute and long-term medical needs of patients, monitoring and maintaining continuity of care between physician visits, and triaging nursing and family telephone calls. This individual will work closely with physicians to develop and implement treatment plans, educate patients and families on health and illness prevention, and recommend community resources.
Requirements
To be successful in this role, you will need to have a minimum of three years of previous patient care experience in a clinic setting, plus two years of Advanced Practice Nurse or Physician Assistant experience. You must also be licensed as a Registered Nurse and Advanced Practice Nurse in the State of Tennessee, or currently licensed to practice as a Physician Assistant in the State of Tennessee.
