Job Summary:
The Hamlet Rehabilitation and Healthcare Center at Nesconset is seeking a skilled and compassionate Registered Nurse (RN) to join our team in the Adult Day Care department. As a key member of our healthcare team, you will provide high-quality nursing care to our residents, ensuring their physical, emotional, and social needs are met.
Responsibilities:
- Provide nursing services and evaluate the need for nursing care and coordinate continuing services for our residents.
- Coordinate program activities and personnel assignments to ensure seamless delivery of care.
- Develop and implement health education programs for residents and caregivers, promoting healthy lifestyles and disease prevention.
- Monitor residents' health status through skilled and non-skilled nursing interventions, ensuring timely and effective care.
- Document resident care plans, medication records, and treatment plans accurately and thoroughly.
- Participate in quality improvement strategies to enhance the overall quality of care.
- Supervise and mentor staff members, providing guidance and support to ensure excellent patient care.
- Coordinate with physicians, medical services, and other healthcare professionals to ensure comprehensive care.
- Maintain accurate and compliant health records and general records according to our facility's policies and procedures.
Requirements:
- Graduate of an accredited school of nursing.
- Registered Nurse (RN) licensure.
- Experience in adult day care or similar healthcare setting, with strong leadership skills and clinical competence.
- Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
- Hours: 8am - 4pm, with occasional flexibility to meet the needs of the department.
