We are seeking a dedicated and compassionate Pediatric Private Duty LPN/RN Nurse to join our team at Phoenix Home Care & Hospice in Chatham, IL. As a Private Duty Pediatric Nurse, you will have the opportunity to make a meaningful difference in the lives of children and families in our community.
Responsibilities:
- Assist in the development, review, and revision of the plan of care.
- Provide nursing care in accordance with the physician's plan of treatment, individual plan of care, as authorized by the client's payer source.
- Observe, record, and report reactions to treatment and any changes in the client's condition to appropriate personnel and/or physician.
- Provide ongoing instruction and supervision of the client, family, or other health team personnel in the plan of treatment.
- Monitor and record the patient's condition and document provided care services via electronic charting.
- Participate in education programs, quality assessment activities, nurse's meetings, and other staff meetings as required.
Requirements:
- Active Licensed Practical Nurse or Registered Nurse License.
- Problem-solving skills and ability to multi-task.
- Excellent Communication Skills.
- Positive, Professional attitude.
