About the Job: We're seeking a compassionate and skilled Pediatric Care Specialist to provide high-quality care services in the comfort of their own homes. As a member of our team, you'll be responsible for delivering individualized nursing care to patients based on their unique needs and treatment plans.
Responsibilities:
- Develop and implement personalized care plans for each patient.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
- Provide ongoing education and support to patients and their families.
- Document all care services provided via electronic charting.
- Participate in education programs, quality assessment activities, and staff meetings as required.
Requirements:
- Active Licensed Practical Nurse or Registered Nurse License.
- Problem-solving skills and ability to multi-task.
- Excellent Communication Skills.
- Positive, Professional attitude.
