We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team at the Aberdeen Community Based Outreach Clinic (CBOC) in Aberdeen, SD. As an LPN, you will work under the direction of a Registered Nurse (RN) as a member of a Primary Care team, providing basic and specialized nursing care for adult and geriatric patients with a variety of physical, mental, behavioral, or psychological needs.
Responsibilities- Provide basic and specialized nursing care for adult and geriatric patients
- Work under the direction of an RN as a member of a Primary Care team
- Assist with procedures, administer vaccines, and perform focused assessments and evaluations
- Collaborate with medical providers, social work, and case management on patient care needs
- Carry out assigned patient care based on the patients' conditions
- Observe, identify, and respond to the patient's needs for care
- Prepare and administer prescribed medications
- Document patient information in the electronic health record
- Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or the Accreditation Commission for Education in Nursing
- Full, active, current, and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth of the United States, or District of Columbia
- Ability to work effectively with patients, family members, and other staff
- Knowledge of human behavior, patient motivations, and reactions to situations
- Ability to establish constructive relationships with individual patients and their families
This is a full-time position with a work schedule of Monday - Friday, 8:00am - 4:30pm. The selected applicant will be required to complete an online onboarding process and pass a pre-employment physical examination or evaluation.
