Pharmacist in Ambulatory Care

The Christ Hospital Cardiovascular Associates is looking for a highly motivated Pharmacist in Ambulatory Care to join our team. This role will involve developing and implementing pharmacy-related processes and procedures within our ambulatory care setting. As an essential member of our team, you will work closely with healthcare providers to manage patient health outcomes, develop effective medication regimens, and provide assistance with polypharmacy issues.
Key Responsibilities:
- Assist care managers with patients in the Transitional Care Management (TCM) process who have significant changes to previous medication regimens and/or medication discrepancies
- Work with patients to ensure understanding of medication plans, special instructions, and identify barriers to taking prescribed medications
- Collaborate with providers to clarify medication therapy plans
- Review data to identify patients on high-risk medications and work with providers to develop safe medication regimens
Qualifications:
- Doctor of Pharmacy (Ph.D.) degree
- Significant clinical experience with completion of a PGY2 residency in ambulatory practice/oncology or equivalent experience
- Licensed to practice pharmacy in the State of Ohio
Estimated Salary: $120,000 - $140,000 per year
