Job Summary

Natchitoches Regional Medical Center is seeking a highly skilled Clinical Liaison to facilitate seamless transitions for patients discharging from a facility setting to post-acute care. The ideal candidate will possess excellent communication and organizational skills, with the ability to work independently and as part of a team.

Key Responsibilities
  1. Respond to patient referrals within an hour of notification, ensuring timely and efficient transitions.
  2. Conduct thorough assessments of patients to determine the level of care required, utilizing a comprehensive head-to-toe functional assessment.
  3. Verify patient information, including address, phone number, and payer details, to ensure eligibility criteria are met.
  4. Coordinate the transfer of orders and referral information to applicable agencies or facilities, ensuring seamless continuity of care.
  5. Collaborate with the Medical Director to review patient assessments and ensure accurate and comprehensive care planning.
  6. Notify referral sources of acceptance or denial within 2 hours of receiving the referral, maintaining open communication and transparency.
  7. Develop and implement effective census growth plans to achieve budgeted admit goals, utilizing data analysis and market research.
  8. Participate in daily staff meetings to ensure patient satisfaction and validate continuity of care, fostering a culture of collaboration and open communication.
  9. Attend meetings with staff, DON, Medical Director, and Medical Advisory Board, staying informed and engaged in facility initiatives and process improvements.
  10. Provide educational in-services to effectively communicate the features, benefits, and specialty programs of Natchitoches Regional Medical Center, promoting exceptional patient outcomes and transitions of care.
Requirements
  1. Current LPN or RN license in the state of practice.
  2. One (1) year acute care hospital experience.
  3. Independent means of transportation, valid driver's license, and automobile liability insurance coverage.
Preferred Qualifications
  1. Previous Clinical Liaison experience.
