Nurse Care Specialist

We are seeking a skilled Registered Nurse to join our team at IntelyCare. As a Nurse Care Specialist, you will provide high-quality patient care in post-acute facilities, collaborating with medical staff and residents to develop care plans.
Key Responsibilities:- Providing direct patient care, including administering medications and treatments
- Collaborating with healthcare teams to develop and implement individualized care plans
- Monitoring patient outcomes and making adjustments to care plans as needed
- Communicating effectively with patients, families, and healthcare teams
- Participating in quality improvement initiatives to enhance patient care and outcomes
- Current Delaware state license to practice as a Registered Nurse (RN)
- Diploma from an accredited nursing program
- Minimum 1 year of experience as a Registered Nurse or in a related role
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
