Job Description

We are scouting for skilled clinical professionals to join our national expansion efforts. As a Clinical Care Specialist, you will be responsible for providing high-quality, patient-centered primary care services to patients residing in skilled nursing facilities (SNFs). This role requires rounding and managing the clinical care of the assigned SNF population, prescribing medications, and treatments as needed.

Essential Functions & Responsibilities

Rounding daily to assess patients' needs and provide timely interventions.Prescribing medications and treatments based on individualized care plans.Providing comprehensive primary care services, including documentation and communication with facility nursing staff.Participating in phone/secure messaging consultations with facility nursing staff to address patient concerns.Maintaining accurate and timely documentation of all patient encounters using Theoria's proprietary electronic health record system, ChartEasy.Familiarity with common EHR systems used in SNFs, including PointClickCare, MatrixCare, and American HealthTech.Avoidance of late arrivals or absences, ensuring reliable transportation to scheduled shifts.
