We are seeking a highly skilled and experienced Lead Care Coordinator to join our team at Trinity Health. As a key member of our Utilization Review and Management team, you will be responsible for overseeing the daily caseloads of our UR/UM teams, ensuring that our patients receive the highest level of care.
Key Responsibilities- Develop, implement, and monitor administrative and clinical processes to ensure staff activities are focused on achieving our program goals.
- Coordinate assignment of daily work, ensuring work is completed within required timeframes and examines work product to ensure it meets department clinical and process guidelines.
- Provide feedback to staff on performance, identifying performance improvement opportunities and working collaboratively with staff to develop and implement remediation plans.
- Bachelor's Degree in Nursing or related field.
- 3-5 years of previous job-related experience, with leadership experience preferred.
- Current Registered Nurse License in the State of Illinois.
