LPN Supervisor

We are seeking a skilled LPN Supervisor to join our team at Ontario Center for Rehabilitation and Healthcare. As a key member of our healthcare team, you will be responsible for overseeing the daily operations of our nursing home and ensuring the highest level of care for our residents.

• Conduct regular facility rounds to monitor resident care and identify areas for improvement
• Supervise the work of aides to ensure proper workflow and efficient use of resources
• Monitor medication assistance times to ensure accurate recording and follow-up as needed
• Intervene in resident conflicts and behavior issues to maintain a safe and respectful environment
• Facilitate communication among healthcare staff to ensure timely addressing of medical and mental health situations
• Investigate and document facility incidents and accidents, providing a summary to the EHP Coordinator/Administrator
• Assist with late admissions and re-admissions, including paperwork and medication changes
• Communicate shift activity through 24-hour reports to be reviewed by the Enriched Housing Program Coordinator/Administrator

• Ability to work effectively as a team member
• Valid state LPN license
• Good standing with the State Registry
