The Pharmacogenomics Nurse Case Manager is a critical role within our organization, responsible for delivering high-quality care management and coordination services to our members. This position requires a skilled and compassionate nurse who can effectively partner with healthcare providers, pharmacies, labs, payers, and community resources to ensure seamless care delivery.
Key Responsibilities- Provide pre-authorization for testing and retrieve test results
- Monitor and track the continuation of care
- Develop and execute programs to improve care delivery
- Lead process improvement projects
- Provide leadership and influence others to meet member needs
- Bachelor's degree in nursing (BSN) from an accredited US University
- Minimum two years of medical-surgical experience in an acute care setting
- Three years of nursing experience in medical case management, ambulatory nursing, or community/homecare experience
- Active RN license from the state licensing board
- Case Management certification (preferred)
