Comprehensive Cancer Care Coordinator

At USA Health, we are seeking a dedicated and compassionate Comprehensive Cancer Care Coordinator to join our multidisciplinary team at the Mitchell Cancer Institute.
Job Summary:
We offer a competitive salary of $95,000 per year, commensurate with experience.
About the Role:
This is a unique opportunity for an experienced Oncology Nurse Navigator to take on a comprehensive care coordination role, ensuring seamless patient journeys from screening to survivorship. The ideal candidate will be an advocate, interpreter, educator, and counselor for oncology patients and their families.
Responsibilities:
- Coordinate care services for patients across the cancer continuum
- Promote timely access to care and reduce emergency room visits by fostering effective communication among healthcare providers
- Develop and implement strategies to increase utilization of preventative care services
- Represent the multidisciplinary team as an advocate, interpreter, educator, and counselor, providing expert guidance and support to patients and families
- Create and promote adherence to treatment and survivorship care plans, empowering patients to take an active role in their care
- Collaborate with patients to plan and monitor care, ensuring informed decision-making and optimal outcomes
- Document patient teaching and respond to patient requests for information in a timely and empathetic manner
- Provide emotional support and guidance to families during challenging decision-making periods
- Cultivate strong relationships with primary care and specialty providers to facilitate co-management and ensure cohesive care delivery
- Identify and assist high-risk patients in need of additional support or resources
Requirements:
- Bachelor's degree in nursing from an accredited institution
- A minimum of five years of professional nursing experience, with three years focused in oncology
- OR graduation from an accredited school of nursing meeting minimum state standards, seven years of professional nursing experience, with five years in oncology
- Current licensure as a registered nurse in the state of Alabama
- ACLS certification within six months of employment
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders
Preferred Qualifications:
- Experience in care coordination, patient navigation, and/or case management
