  • Advocate for clients or patients to resolve crises and provide support
  • Refer patients, clients, or families to community resources to assist in recovery from mental or physical illness
  • Plan discharge from care facilities to home or other care facilities

Requirements:

  • Masters degree in Social Work, Psychology, or Mental Health Counseling (BSW considered)
  • State registration required
  • Minimum 1-3 years' experience in Social Work/Case Management/Counseling
  • Hospital experience preferred
