Join Our Team as a Care Coordinator

At Person Centered Services, we are dedicated to providing exceptional care and support to individuals with intellectual and developmental disabilities. We are seeking a passionate and dedicated Care Coordinator to join our team and make a meaningful difference in the lives of our clients.

About the Role

The Care Coordinator will be responsible for coordinating and advocating for all aspects of the individual's care, including health and behavioral healthcare, community supports, and other services required to meet their needs. This role requires a person-centered, holistic approach, where each individual's wants and needs are met through collaboration with their healthcare and service providers.

Key Responsibilities
  • Develop a comprehensive, person-centered Life Plan with the individual and their circle of support, as well as their entire service provider team.
  • Effectively manage a tiered caseload, while tailoring services to individual needs.
  • Complete enrollment and eligibility documentation.
  • Meet with individuals in their homes, physician/provider offices, and other public places to conduct assessments and provide services.
  • Commit to a respectful, just, and supportive environment for individuals and coworkers, aligning with our commitment to diversity, equity, and inclusion.
Requirements
  • Bachelor's degree with 2 years of relevant experience OR a Licensed Registered Nurse with 2 years relevant experience OR Master's degree with 1 year of relevant experience.
  • Minimum of a Bachelor's degree is required by OPWDD in NYS.
  • Relevant experience consists of providing direct care or support, coordination, and/or advocacy for anyone.
