SUMMARY:
This role involves delivering counseling, case management, and various chemical dependency services tailored to address the social and emotional requirements of individuals experiencing chronic homelessness. The position requires collaboration with local agencies to facilitate housing retention for participants.
ESSENTIAL DUTIES & RESPONSIBILITIES:
- Manage an individual caseload of single adult participants.
- Provide supportive counseling to clients within Chemical Dependency Services, employing evidence-based practices in addiction treatment.
- Engage in the formulation of treatment plans, setting both short-term and long-term objectives.
- Establish connections with community resources, offering daily living skills training, counseling, and compliance documentation.
- Participate in clinical team meetings to review individual cases and overall client progress.
- Maintain communication with all relevant agencies to ensure adherence to service plans.
- Document all required information, including progress notes and treatment plans, in a timely manner.
- Identify program needs and propose enhancements.
- Provide crisis intervention and stabilization support as necessary, which may include after-hours follow-up.
- Perform additional tasks as assigned.
EXPERIENCE, EDUCATION AND SPECIAL REQUIREMENTS:
Must possess current NYS certification as a Credentialed Alcoholism and Substance Abuse Counselor. A Bachelor's degree in a Human Services field is preferred, along with two years of experience in a similar environment, particularly with substance abuse, mental health, and homeless populations. Bilingual Spanish skills are advantageous. Flexibility to work some evenings and occasional weekends is required. Strong writing and oral communication skills are essential, along with proficiency in computer software for document preparation and data management. Experience in crisis intervention techniques and strength-based approaches is necessary. A valid driver's license and personal vehicle are required.
