About the Role

We are seeking a highly skilled Nurse Case Manager to join our team at Paradigm, a leading accountable specialty care management organization. As a Field Nurse Case Manager, you will play a critical role in improving the lives of individuals with complex injuries and diagnoses.

Key Responsibilities
  • Assess, plan, coordinate, implement, and evaluate the medical case management of work-related injuries.
  • Work closely with insurance carriers, medical care providers, attorneys, employers, and employees to ensure the best possible outcomes for injured workers.
  • Monitor the progress of injured workers and report results back to employers and insurance carriers.
Requirements
  • Current, unencumbered Registered Nurse (RN) license in Michigan.
  • Experience in case management of workers' comp cases, preferred.
  • Recommended certifications (CCM, CRRN, CDMS, CRC) preferred.
  • Skills required for success: organization and timeline adherence skills, PC and technology skills, communication skills (written, verbal, and interpersonal).
  • Ability to operate autonomously with minimal oversight.
  • Skilled at patient education.
  • Bilingual Spanish strongly preferred.
  • Valid Driver's License, and good driving history.
