Job Summary

We are seeking a highly skilled and compassionate Outpatient Therapist to join our team at Communicare. As a key member of our treatment team, you will provide evidence-based services to individuals in need of mental health support.

Responsibilities
  1. Provide High-Quality Services: Deliver evidence-based services while maintaining fidelity to promote continuity of care for the individual served.
  2. Medication Management: Arrange for medication evaluation, management, and monitoring to ensure optimal patient outcomes.
  3. Clinical Supervision: Appropriately utilize and accept clinical supervision to provide the highest quality mental health care possible.
  4. Caseload Management: Maintain an active caseload as directed by the supervisor, ensuring timely and effective service delivery.
  5. Compliance and Documentation: Meet all Communicare, Department of Mental Health, and Division of Medicaid standards and policies regarding patient care and documentation.
  6. Productivity and Efficiency: Provide timely services and documentation as directed and meet productivity requirements, working efficiently in Communicare's Electronic Health Record system.
  7. Training and Development: Attend and conduct meetings and training as assigned, successfully completing training in crisis intervention, suicide prevention, and other relevant areas.
  8. Data Collection and Maintenance: Assist in collecting and maintaining data as needed or required, serving as a member of Communicare's treatment team.
  9. Rapid Response and Intake Services: Provide rapid response crisis services, intakes, and therapy services as directed by the supervisor.
  10. Family and Community Involvement: Assess home/family situations as it relates to the client's needs and work directly with the Family to ensure that the family is participating in treatment.
  11. Collaboration and Referral Development: Participate in conferences and meetings with referral sources or other outside agencies pertinent to the individual treatment plan, working closely with other programs in Communicare to maintain continuity of care.
  12. Community Outreach: Establish and maintain referral bases within the community.
Requirements
  1. Education and Licensure: Master's Degree and license or certification in counseling, psychology, social work, marriage and family therapy, or other related behavioral health care fields preferred. If not already licensed or certified, must be eligible for licensure or Department of Mental Health certification within thirty days.
  2. Background Check and Drug Screen: Pass a Communicare background check and drug screen, as Communicare is a drug-free workplace.
  3. Ability to Perform Job Responsibilities: Ability to perform the responsibilities of the position as described above.
  4. Independence and Interpersonal Skills: Ability to work independently and relate positively to individuals who are referred to Communicare, their families, other staff members, members of the community, and staff from other agencies.
  5. Reliable Transportation: Must have reliable transportation and the ability to travel throughout the catchment area and to trainings and meetings in other locations outside the catchment area as needed, with a valid driver's license, automobile insurance, and an insurable driving record.
