Position Summary

Conway Medical Center's Home Hospice CNA provides essential support to patients facing health challenges. You will deliver compassionate care, assisting with daily living activities, providing emotional comfort, and adhering to individual patient needs outlined in their comprehensive plan of care.

Job Duties

  • Assist patients with all aspects of daily living, including ambulation, turning, repositioning, and promoting comfort.
  • Provide assistance with personal hygiene tasks such as bathing, dressing, dental care, and managing bowel and bladder functions.
  • Support patients with walking, exercising, and transferring in and out of bed. Assist with lifting, moving, positioning, and transporting residents as needed.
  • Manage catheter care and incontinence care according to established protocols.
  • Utilize adaptive equipment as directed by the plan of care or facility policies.
  • Monitor skin integrity and promptly report any impaired skin, injuries, or concerns to the nurse.
  • Perform light housekeeping tasks such as making beds and maintaining a clean environment.
  • Demonstrate empathy and understanding in handling the emotional needs of patients and their families facing illness and end-of-life situations.
  • Gain knowledge of patient-specific dietary requirements, preferences, and meal routines.
  • Accurately record patient information, including meal consumption and other relevant observations.
  • Report any incidents, accidents, changes in condition, or safety concerns to the nurse promptly.
  • Communicate effectively with the Case Manager regarding daily patient care needs and concerns.
  • Maintain awareness of abuse recognition and reporting procedures.

Qualifications

  • High school diploma or equivalent required.
  • Previous experience as a CNA in hospice or related healthcare setting is preferred.
  • Current South Carolina State Nursing Assistant Certification (CNA) in good standing is mandatory.
  • Valid South Carolina Driver's License required.

Job Expectations

  • Ability to work flexible hours and travel between various locations as needed.
  • Possess a valid driver's license and insured vehicle that meets state requirements.
  • Maintain a professional appearance and adhere to the organization's dress code.
  • Demonstrate strong communication and interpersonal skills.
