About the Role

The Case Manager will be responsible for coordinating and directing care specific to the needs of each patient. This will involve collaborating with the Rehabilitation Program Director and Facility Case Management Director to ensure seamless care delivery. You will also be responsible for ensuring the implementation of the patient's treatment plan, facilitating patient involvement throughout the rehabilitation process, and documenting findings of the Discharge Planning Evaluation and psychosocial assessments.

Requirements
  • Three years' clinical, hospital nursing experience required
  • Preference given to those with 2 years of case management experience or 2 years of critical care experience
