Job SummaryThe successful candidate will have a strong background in nursing, with a focus on heart failure care. You will be responsible for assessing, planning, implementing, and evaluating patient care, as well as collaborating with other healthcare professionals to ensure seamless transitions and optimal patient outcomes.
Responsibilities- Provide direct patient care, including assessment, planning, implementation, and evaluation of care
- Collaborate with other healthcare professionals to ensure seamless transitions and optimal patient outcomes
- Participate in interdisciplinary rounds to facilitate appropriateness of care and smooth transitions across the continuum
- Actively participate in quality improvement initiatives to improve patient care and outcomes
- Develop and maintain a culture of safety, respect, and open communication among team members
- Current RN licensure in the state of Georgia
- Associate's degree in nursing or diploma in nursing
- At least 1 year of RN experience, with a focus on heart failure care
- Strong communication and interpersonal skills
- Ability to work collaboratively as part of a team
- BSN degree
- Experience working in a cardiac care unit
- Basic computer skills, including proficiency in electronic medical records
