Job Summary

We are seeking a skilled Patient Education Specialist to join our team at Trinity Health Corporation. In this role, you will provide expert diabetes education and training to clients, assisting with the development of diabetes services that meet their unique needs.

About the Role

This full-time position is responsible for assessing patients' needs, providing individualized education, and coordinating follow-up services. You will work closely with physicians, healthcare teams, and patients to ensure seamless care coordination and excellent patient outcomes.

Key Responsibilities
  • Provide comprehensive diabetes education and training to patients in individual and group settings
  • Conduct initial needs assessments to identify areas for improvement
  • Develop and implement personalized education plans tailored to each patient's age, reading level, language, and disability
  • Coordinate and provide follow-up services, including phone calls and in-person visits
Requirements

To be successful in this role, you will need:

  • A minimum of 2 years of medical/surgical nursing experience, with a focus on patient education and diabetes management preferred
  • A BSN degree is highly desirable
