Nurse Case Manager PRN Position

The Case Manager (CM) plays a vital role in ensuring high-quality, patient-centered care by managing rehabilitative care to achieve optimum outcomes. This position requires a collaborative approach, working closely with the Rehab Program Director and Facility Case Management Director to coordinate departmental functions.
Key Responsibilities- Provide program orientation to patients, families, and caregivers, including the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
- Act as the coordinator of patient, family, and caregiver education, promoting participation in team discussions related to plans, goals, and status.
- Ensure the implementation of the patient's treatment plan, supporting the patient's strengths, abilities, needs, and preferences.
- Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient, family, or caregiver.
- Assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.
- Associate Degree in Nursing or Nursing Diploma required.
- Bachelor's Degree in Nursing preferred.
- 2+ years of experience in case management or 3+ years of experience in clinical nursing required.
- InterQual experience preferred.
- Current licensure as a Registered Nurse in the state(s) of practice according to law and regulation required.
- Certification in Case Management preferred.
