We are seeking a highly skilled Pharmacist to join our team at Fairview Health Services. As a Medication Therapy Management (MTM) Pharmacist, you will play a critical role in providing comprehensive medication management services to patients.
Key Responsibilities- Provide Medication Therapy Management to patients, ensuring all drugs are appropriately indicated, effective, safe, and convenient.
- Obtain patient histories and apply them, along with other patient data, in clinical evaluations.
- Identify, resolve, and prevent medication therapy problems.
- Evaluate signs and symptoms relative to disease processes and make recommendations and/or modifications of medication therapy.
- Ensure patient understanding of therapy goals and work with providers to achieve these goals.
- Evaluate and document patient outcomes at each encounter.
- B.S. Degree in Pharmacy or PharmD from an accredited school of pharmacy.
- Current Minnesota pharmacy license.
- Certification in Pharmaceutical Care as approved by Fairview Pharmacy Services Management.
- Basic Life Support Certification.
- Preferred: Ambulatory Pharmacy Board Certification (BCACP) or other accredited board certification.
- Minimum 3 years of experience as a practicing pharmacist in a clinical setting or PGY1 residency with an ambulatory focus.
