As a Home Health Physical Therapist with BAYADA, you will be part of a collaborative multidisciplinary team that prioritizes patient-centered care and provides opportunities for professional growth and development.

Your primary responsibilities will include:
- Making home visits to clients in designated geographic territories, providing individualized care plans tailored to their specific needs
- Performing diagnostic tests and measurements, such as mobility assessments, strength evaluations, and balance testing
- Implementing integumentary protective interventions to prevent wound formation and promote wound repair
- Developing and implementing appropriate physical therapy care plans, including manual therapeutic exercises, gait training, and other interventions
- Continually assessing and revising client care plans, collaborating with nursing staff as needed
- Educating clients, family members, or caregivers on rehabilitative care and activities necessary for independent living
To be considered for this role, you must have:
- A current state license to practice as a Physical Therapist (PT)
- A minimum of one year of recent work experience as a Physical Therapist
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Strong interpersonal, organizational, and problem-solving skills
- Solid computer skills, with prior experience with electronic medical records (EMR) preferred
