Position Overview
The Licensed Practical Nurse (LPN) operates under the guidance of the Physician, Nurse Supervisor, and/or Practice/Office Manager. This role is essential in identifying patient needs and symptoms proactively. The LPN provides care as directed by the physician, which includes preparing patients for examinations, assisting during medical procedures, coordinating diagnostic tests, and ensuring follow-up care is managed effectively. Documentation of treatment in the medical record is also a critical responsibility. The LPN plays a vital role in patient and family education, demonstrating the necessary competencies and skills to care for diverse patient populations, including neonates, infants, children, adolescents, adults, and the elderly. Additional duties may be assigned as needed. All LPNs are expected to maintain licensure, possess comprehensive knowledge, and adhere to the nursing practices outlined by the Georgia Professional Nurse Practice Act, as well as the Nursing Practice Standards and Code established by the National Federation of Licensed Practical Nurses, Inc. (NFLPN). As a member of the patient services team, the LPN is expected to uphold patient advocacy, adhere to system policies and procedures, and contribute to service excellence goals.
Key Responsibilities and Essential Functions
Professional Practice and Accountability
A. Adheres to the scope of practice and standards set forth by the NFLPN.
B. Engages in health promotion through clinical care, employing evidence-based practices and critical thinking to ensure individualized patient care and effective coordination of services.
C. Implements current clinical practice standards.
Teamwork and Collaboration
A. Ensures the delivery and documentation of safe, high-quality patient care that aligns with the professional care delivery model. This includes diverse interdisciplinary communication methods, timely documentation, and fostering a respectful, inclusive clinical environment.
B. Demonstrates teamwork by practicing in a manner that is financially responsible and supports the recruitment and retention of healthcare team members.
Professional Development and Initiative
A. Completes all initial and ongoing competency assessments, mandatory education, and licensure requirements.
B. Acts as a preceptor and/or mentor for other practical nurses and unlicensed staff, ensuring a qualified workforce and promoting a healthy work environment.
C. May delegate tasks to unlicensed staff as appropriate within their scope of practice.
Evidence-Based Practice and Research
A. Advocates for evidence-based practice, nursing research, and performance improvement initiatives.
B. Complies with all healthcare system policies, procedures, and clinical competencies, including legal and accreditation standards.
C. Participates in data collection and formulates relevant clinical questions to enhance evidence-based practice.
Operational Duties
A. Provides direct patient care as directed by the healthcare provider.
B. Proactively plans for patient care across the continuum through pre-visit planning processes.
C. Contributes to financial viability through service quality and clinical excellence.
D. May serve as a liaison with nursing leadership regarding clinical operations and quality initiatives.
Patient Advocacy and Liaison
A. Delivers relationship-based, patient-centered care that respects population-specific characteristics in a culturally sensitive manner.
B. Collaborates with patients, their families, and significant others to create a comfortable and safe environment for care delivery, ensuring that the patient's goals are central to the care plan.
Required Minimum Education
Graduate from an accredited practical or vocational nursing program.
Required Licenses and Certifications
Basic Life Support Certification - Required
Licensed Practical Nurse License - Required
Preferred Experience
Previous healthcare experience is preferred, particularly in an ambulatory or outpatient setting. Experience in a hospital-based environment may be considered.
Required Skills
Must possess strong organizational skills and manage time effectively to ensure patient comfort and satisfaction. Attention to detail and strong interpersonal skills are essential for positive interactions with staff, providers, and patients. Proficiency in verbal and written English is required, along with basic computer skills to support clinical care. Adherence to the WellStar Credo and Code of Conduct is expected.
