Job Summary

We are seeking a skilled and compassionate Pediatric Home Health Care Professional to join our team at UnityPoint Health. As a Pediatric Home Health Care Professional, you will work one-on-one with children and their families to develop and implement personalized care plans that meet their unique needs.

Responsibilities
  • Provide skilled nursing care to pediatric patients with special healthcare needs in their homes
  • Perform prescribed activities under the direction of the patient's physician, including patient assessments, feedings, respiratory system management, medication administration, and activities of daily living
  • Assist in care coordination with the care team and may be responsible for transportation of patients
Qualifications
  • Graduate of a State Board-approved program for Licensed Practical Nurses or Registered Nurses and valid license as Licensed Practical Nurse or Registered Nurse
  • Strong computer skills and ability to understand and apply guidelines, policies, and procedures
  • Maintenance of an acceptable driving record and ability to work as a collaborative team member
  • CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with re-certification
