Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team as a Resource Nurse. In this role, you will provide nursing care to patients and families in their homes, hospitals, and other settings. You will work collaboratively with our interdisciplinary team to develop and implement individualized plans of care that meet the unique needs of each patient.

Key Responsibilities

    • Evaluate the effectiveness of interventions and make adjustments as needed.
  • Plan of Care Development and Implementation
    • Develop and implement individualized plans of care that address patients' needs and goals.
    • Collaborate with patients, families, and healthcare providers to ensure seamless care transitions.
    • Delegate tasks and responsibilities to other healthcare team members as needed.
  • Communication and Documentation
    • Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions.
    • Document patient care and outcomes in a timely and accurate manner.
Requirements
  • Education and Experience
    • Associate's or Bachelor's degree in Nursing required.
    • One year of nursing experience preferred.
  • Certifications and Licenses
    • Current, unrestricted Registered Nursing License in the state of employment required.
  • Skills and Abilities
    • Ability to develop and implement individualized plans of care that address patients' needs and goals.
    • Ability to communicate effectively with patients, families, and healthcare providers.
    • Ability to work collaboratively with other healthcare team members.
