About the Role:

We are seeking a highly skilled and compassionate Senior Geriatric Care Specialist to join our clinical operations team. As a key member of our healthcare team, you will be responsible for providing direct patient care, including geriatric assessments, diagnosis, treatment planning, and health education.

The ideal candidate will have strong critical thinking and problem-solving skills, excellent communication and interpersonal skills, and the ability to work well under pressure. You must be caring and empathetic, with great listening skills, and able to pay close attention to patient charts, medications, and follow-up on details of patient care.

Key Responsibilities:
  • Provide geriatric assessments, diagnose, and develop treatment plans for patients
  • Collaborate with healthcare teams to ensure comprehensive patient care
  • Order and interpret laboratory and diagnostic studies, prescribe medication, and refer patients for consultation when necessary
  • Document patient progress through in-depth notes and summaries
  • Participate in patient care rounds and conferences to ensure successful patient outcomes
Requirements:
  • AARNP or similar advanced degree in Nursing required
  • Current Nurse Practitioner Certification in the State of practice required
  • Board certification by AANP or ANCC preferred but may be required for certain States
  • A minimum of 1 year of clinical experience preferred, but willing to consider strong new graduates
