Responsibilities:
  • Provide skilled nursing care to patients with complex wound dressings, rehabilitation, and tube feedings or rapidly changing health status.
  • Work collaboratively with the healthcare team to develop and implement individualized care plans.
  • Administer medications and treatments as ordered by the physician.
  • Monitor patients' vital signs and report any changes to the physician.
  • Document patient care and progress in the medical record.
Requirements:
  • Current LPN licensure in the state of New York.
  • At least 1 year of experience as an LPN in a skilled nursing setting.
  • Ability to work independently and as part of a team.
  • Strong communication and interpersonal skills.
