The Medication History Technician will play a vital role in ensuring the accuracy of patient medication lists. This position requires strong communication skills and the ability to work effectively with healthcare teams to obtain and verify medication information. As a Medication History Technician, you will be responsible for obtaining accurate home medication histories, processing patient medication data, and updating electronic records. Your attention to detail and ability to work in a fast-paced environment will be essential in this role.

Key Responsibilities:

  • Obtain accurate home medication histories through chart review, patient interviews, and phone calls to pharmacies and physician offices.
  • Process patient medication data and enter information accurately into the computer system.
  • Update electronic home medication databases and compare these lists to orders written upon admission to the hospital.
  • Work with nursing, pharmacists, and physicians to resolve any discrepancies.
  • Collect and enter complete allergy information, including types of reactions, into the computer system.
  • Utilize medication information resources to identify medications as needed.
  • Collect data for reporting to hospital quality committees.
  • Perform other duties as assigned, including coverage in the pharmacy.

Requirements:

  • Virginia Board of Pharmacy technician licensure or Licensed Practical Nurse in the Commonwealth of Virginia required.
  • Current PTCB certification preferred.
  • A minimum of two years of Pharmacy Technician or LPN experience required.
  • Strong mathematics and interpersonal skills required.
  • Ability to work with computers.
  • Ability to work in a versatile environment and manage multi-task demands.
