Key Responsibilities:
- Conduct comprehensive assessments to identify participant needs and preferences.
- Develop and implement tailored support plans, incorporating participant goals, interests, and abilities.
- Collaborate with family members, day programs, employment programs, and other team members to ensure seamless support delivery.
- Communicate effectively with participants, families, and colleagues to share progress, concerns, and suggestions.
Requirements:
- Candidates must be at least 18 years old and possess a High School Diploma or GED equivalent.
- A valid driver's license with a positive driving abstract is required.
- The ideal candidate will have at least one year of experience working with individuals with developmental disabilities.
- CPR certification is mandatory, with training and recertification provided by the employer.
- Schedule flexibility, including evenings, overnights, and weekends, is essential.
