About the Role

Pawnee Mental Health Services is seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will play a vital role in assessing the needs of adult clients with Severe and Persistent Mental Illness and developing a plan to enhance their functioning.

Key Responsibilities

  • Collaborate with the treatment team to identify treatment goals and strategies
  • Establish and coordinate treatment services by liaising with community organizations
  • Build relationships with clients and their families to support informed decision-making
  • Document plans and updates in the electronic health record

Additional Responsibilities

  • Provide education and resources to clients, families, and the community about mental illness
  • Assist clients in maintaining benefits to support their rehabilitation process

Requirements

  • Bachelor's degree in a human services field or equivalent work experience
  • Reliable transportation, a clean driving record, and automobile liability insurance
  • Excellent communication skills, both written and verbal
  • Basic computer skills

Preferred Qualifications

  • Knowledge of the needs and challenges of adults with mental illness
  • Experience with Microsoft Office
