MDS Coordinator

Key Responsibilities:
- Monitor and ensure accurate charting and reimbursement for patients.
- Prepare admission paperwork and verify that the Electronic Medical Record is completed by the designated interdisciplinary team member.
- Serve as the primary trainer for the MDS and care planning processes.
- Assist in the completion of MDS assessments as directed by the supervisor.
Qualifications:
- Active RN/LPN license in the State of Minnesota.
- Experience with MDS/CAA completion, care planning, and error follow-up.
- Familiarity with Medicare/PDPM requirements and ICD-10 coding.
- Experience in collaborating with finance teams and insurance carriers regarding billing issues and claims.
Preferred Experience:
- Prior long-term care experience.
- Two years of experience for LPN candidates.
- ICD-10 coding experience is advantageous.
Essential Skills:
- Ability to collect, organize, and analyze data effectively.
- Strong independent work ethic with excellent time management skills.
- Collaborative team player.
Accountabilities:
- Ensure the completion of the RAI process for assigned residents.
- Maintain professional development through relevant training and conferences.
- Exhibit professionalism and courtesy in all interactions with residents, families, and staff.
- Adhere to federal, state, and organizational regulations and standards.
- Educate and train staff on the RAI process and related software.
- Engage in quality improvement initiatives within the community.
- Communicate effectively with supervisors and relevant parties regarding important updates.
- Complete MDS, CAAs, and care plans in compliance with regulatory requirements.
- Perform additional duties as assigned by the supervisor.
