Walker Methodist Westwood Ridge is seeking a dedicated RAI Coordinator for our Transitional Care Unit (TCU) department. This role is pivotal in overseeing the documentation and reimbursement processes for residents utilizing Inclusive Insurance and Medicare services.

Key Responsibilities:
  • Monitor and ensure accurate charting and reimbursement for patients.
  • Prepare admission paperwork and verify that the Electronic Medical Record is completed by the designated interdisciplinary team member.
  • Serve as the primary trainer for the MDS and care planning processes.
  • Assist in the completion of MDS assessments as directed by the supervisor.

Qualifications:
  • Active RN/LPN license in the State of Minnesota.
  • Experience with MDS/CAA completion, care planning, and error follow-up.
  • Familiarity with Medicare/PDPM requirements and ICD-10 coding.
  • Experience in collaborating with finance teams and insurance carriers regarding billing issues and claims.

Preferred Experience:
  • Prior long-term care experience.
  • Two years of experience for LPN candidates.
  • ICD-10 coding experience is advantageous.

Essential Skills:
  • Ability to collect, organize, and analyze data effectively.
  • Strong independent work ethic with excellent time management skills.
  • Collaborative team player.

Accountabilities:
  • Ensure the completion of the RAI process for assigned residents.
  • Maintain professional development through relevant training and conferences.
  • Exhibit professionalism and courtesy in all interactions with residents, families, and staff.
  • Adhere to federal, state, and organizational regulations and standards.
  • Educate and train staff on the RAI process and related software.
  • Engage in quality improvement initiatives within the community.
  • Communicate effectively with supervisors and relevant parties regarding important updates.
  • Complete MDS, CAAs, and care plans in compliance with regulatory requirements.
  • Perform additional duties as assigned by the supervisor.
