We are seeking a skilled Nurse Practitioner to join our team in Philadelphia. As a Nurse Practitioner, you will be responsible for providing primary care to patients in skilled nursing facilities.
Key Responsibilities:- Perform primary care in a post-acute setting for short-term and long-term patients
- Manage chronic disease, diagnose illnesses and medical conditions, and implement a plan of care
- Order and interpret lab results
- Prescribe and monitor medications
- Consult with physicians and coordinate patient care with other healthcare practitioners
- Educate patients and their caregivers on coping skills to handle chronic and age-related conditions
- Build relationships with patients, patients' family members, and facility staff to facilitate effective treatment delivery
- Promote healthy lifestyle changes
- A minimum of 1 year experience as a Nurse Practitioner in a primary care setting
- Post-acute care experience a plus, but not required
- Active and unrestricted Pennsylvania NP license required
- Active and unrestricted DEA with prescriptive authority required
