Job Title: Clinic Nurse LPN

At Mid-Delta, we are seeking a skilled and compassionate Clinic Nurse LPN to join our team of healthcare professionals. As a Clinic Nurse LPN, you will play a vital role in providing high-quality patient care and contributing to the success of our community health center.

Job Summary:

The Clinic Nurse LPN will work closely with our medical team to provide direct patient services, perform designated independent procedures, and assist with the delivery of care in accordance with the Patient Centered Medical Home model of care. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Key Responsibilities:
  • Prepare patients for examination, including vital signs, height, weight, and other necessary assessments.
  • Collect labs and administer injections as ordered by the clinician.
  • Make appointments and notify patients of scheduled appointments.
  • Act as a systems navigator and point of contact for patients and families, providing direct access for asking questions and raising concerns.
  • Assemble patient information and provide clinical data to specialists as needed.
  • Triaging patient calls and providing medical advice as necessary.
  • Routing medication refills to the appropriate clinician.
  • Checking the system weekly to ensure labs, diagnostic imaging, and documents are assigned to a provider.
  • Identify and utilize cultural and community resources.
  • Assist providers with healthcare activities, diagnostic procedures, and medication administration.
  • Perform other necessary duties as required by the Clinic Director or Nursing Director.
Qualifications:
  • Certification as a Licensed Practical Nurse (LPN) or Registered Nurse (RN).
  • Sufficient experience to carry out the duties of this position.
  • Basic Life Support (BLS) certification.
  • Ability to relate effectively with patients and providers.
  • Participation in ongoing age-related in-service training.
Evaluation:

The evaluation of work performance will be ongoing, conducted by the physician in charge, and will include specific duties, responsibilities, employee attitudes, and general working behavior. Formal evaluation will normally be performed annually, but may be initiated at other times by the Clinical Nursing Director/Supervisor or the Medical Director.

This is a FLSA Non-Exempt position. This is an OSHA low-risk position. This is an ADA negotiable position. This is a safety-sensitive position. Mid-Delta follows state and federal vaccine recommendations.
