  • Develop and implement individualized plans of care to meet the unique needs of each patient.
  • Conduct thorough assessments and evaluations to identify patient needs and goals.
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordination.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.
Responsibilities:
  • Plan and implement physical therapy interventions to improve patient function and mobility.
  • Conduct patient education and training on proper use of equipment and techniques.
  • Monitor patient progress and adjust treatment plans as needed.
  • Document patient information and treatment plans accurately and thoroughly.
  • Participate in interdisciplinary team meetings to discuss patient care and coordinate services.
Requirements:
  • Current licensure as a Physical Therapist in the state of employment.
  • Minimum of one year of experience as a Physical Therapist in an acute setting.
  • Valid driver's license and auto insurance.
  • Ability to lift and move patients as needed.
