Key Responsibilities
  • Primary Care Rounding: Conduct regular rounds in skilled nursing facilities to provide high-quality patient care.
  • Chronic Disease Management: Manage chronic diseases, diagnose illnesses, and implement plans of care for patients.
  • Clinical Leadership: Collaborate with clinicians, facility leaders, and other stakeholders to ensure effective care delivery and quality improvement initiatives.
  • Patient Relationships: Build strong relationships with patients, their families, and facility staff to facilitate effective treatment delivery.
  • Medical Director Duties: Provide medical directorship for assigned facilities, overseeing clinical operations and ensuring compliance with regulatory requirements.
  • Clinical Oversight: Provide guidance and mentorship to our clinicians, conducting clinical case reviews as needed and sharing best practices for quality care.
  • Care Coordination: Collaborate with leadership to review care coordination initiatives, develop collaborative intervention plans, and share ideas for network management issues.
Requirements
  • At least 1 year of primary care experience; post-acute care experience is a plus.
  • Strong clinical skills and ability to work independently.
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced environment and adapt to changing priorities.
Compensation and Benefits
  • Competitive salary range: $265,000 - $306,000.
  • Comprehensive benefits package, including medical, dental, vision, life insurance, 401(k) with match, and malpractice insurance with tail coverage.
