Job Summary

The Health Services Nurse (LPN) is a critical role within Volunteers of America Mid-States, responsible for delivering high-quality healthcare services to individuals with Developmental Disabilities. As a Licensed Practical Nurse, you will work closely with the Case Manager - RN to implement nursing plans of care, administer medications, and educate individuals and staff on health-related topics.

Key Responsibilities
  • Implement nursing plans of care by administering medications, nursing treatments, and educating individuals and staff as directed by the Case Manager - RN.
  • Schedule and coordinate medical appointments for individuals served in a timely manner and ensure that all specialist recommendations are addressed.
  • Communicate with Primary Care Physicians and specialists to coordinate health services for individuals served.
  • Record all nursing care in records and maintain file systems, as directed by the Health Services Coordinator.
  • Assist in maintaining accurate and up-to-date Health Passports.
  • Effectively work to maintain DMRS standards and take prompt, effective action to correct any deficiencies.
  • Participate as a member of the Circle of Support for individuals served, reporting any concerns to the Case Manager - RN.
  • Be available for shift work and assist in regular household duties.
  • Perform other responsibilities as assigned to meet the mission of the agency.
  • Participate in performance quality improvement (PQI) duties as assigned by supervision and the PQI committee.
Requirements

To be successful in this role, you will need to be currently licensed as a Licensed Practical Nurse with the State of Tennessee and have experience equivalent to one year of nursing, with a focus on caring for individuals with Developmental Disabilities and/or Mental Health concerns. Knowledge of the guidelines established by the state of Tennessee for community support programs for individuals with Developmental Disabilities is preferred.
