We are seeking a dedicated and detail-oriented Referral Coordinator to join our team at El Centro Family Health. As a key member of our patient care team, you will play a vital role in ensuring continuity of care for our patients.

Key Responsibilities:

  • Coordinate referrals for specialty care and other services
  • Ensure compliance with health plan protocols
  • Provide psychosocial support to patients and families
  • Collaborate with healthcare providers to ensure seamless care transitions

Requirements:

  • High school diploma or equivalent
  • Minimum of 2 years' experience in a related field
  • Bi-lingual fluency in English and Spanish
  • Knowledge of eClinicalWorks
