Job Summary

The Clinical Nurse Coordinator (CNC) plays a vital role in delivering high-quality patient care and coordinating all functions in the unit. In partnership with the management team, the CNC monitors and evaluates nursing care in accordance with established policies.

Key Responsibilities
  • Assess, plan, intervene, and evaluate care to positively affect patient outcomes within the scope of nursing practice.
  • Utilize strong clinical judgment and competent or expert nursing skills while following established policies, including those related to confidentiality.
  • Document effectively, succinctly, and timely according to organizational policy.
  • Incorporate time management and prioritize for efficiency and productivity.
  • Follow LGMC process for dispensing of medications and narcotic control and employ clinical judgment related to medication administration.
  • Delegate tasks to other health team members, taking into consideration training, cultural competence, experience, and facility/agency policies and procedures.
  • Adhere to hospital and unit policies, professionally resolving or seeking assistance for clinical problems and conflict.
  • Timely collaborate with the healthcare team to report critical information or change in condition.
  • Question and evaluate practice and evidence through research utilization and experiential learning.
  • Complete annual educational requirements and other specified learning activities on time.
  • Demonstrate professional caring practices through sensitivity, respect, engagement, and assessing for patient and family satisfaction.
  • Create a safe and therapeutic atmosphere for patients, co-workers, other departments, and customers.
  • Promote comfort and prevent suffering through presence and attentiveness.
  • Demonstrate timely pain assessment and reassessment.
  • Coach other members of the team for professional growth and to meet the patient's needs.
  • Promote a culture of safety.
  • Work on behalf of others, representing the concerns of the patient/family/community, including participation in Multidisciplinary Rounds and hourly rounds.
  • Adhere to quality standards such as National Patient Safety standards and infection prevention.
  • Appropriately teach patients and families beginning with admission and continuing throughout hospitalization until the last interaction where instructions for home and/or the next provider are accurately provided.
  • Follow Synergy Professional Practice Model, maintaining and growing competency in response to the needs of patients and the organization.
Requirements
  • One of the following: Registered Nurse License issued by the Department of Health Professions, Commonwealth of Virginia or Registered Nurse with 'multi-state privilege' granted to them by virtue of holding a license in one of the states participating in the Nurse Licensure Compact Agreement and who has not established permanent residency in Virginia.
  • BLS, Basic Life Support Certification from the American Heart Association.
  • ACLS, Advanced Cardiovascular Life Support Certification from the American Heart Association.
  • NRP, Neonatal Resuscitation Program.
  • PALS, Pediatric Advanced Life Support Certification from the American Heart Association.
  • TNCC, Trauma Nursing Core Course from the Emergency Nurses Association.
  • Graduate of a nursing program.
  • Three to Five years of Trauma Specific Nursing experience.
