Key Responsibilities
  • Evaluate patient conditions, develop treatment plans, and provide individualized care to help patients regain independence and functionality.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Document patient progress and outcomes in accordance with regulatory and accreditation standards.
  • Collaborate with nursing staff and families to develop and implement follow-up programs that promote continued progress.
  • Maintain a safe and clean work environment, adhering to company policies and procedures.
  • Perform other duties as assigned, in accordance with the Company's Code of Business Conduct and applicable laws.
Requirements
  • Graduate of an accredited Physical Therapy program.
  • Current and unrestricted Physical Therapy license in the state where services are rendered.
  • Current CPR certification.
  • Strong organizational and communication skills.
