Key Responsibilities

The primary responsibility of the After-Hours Registered Nurse is to ensure the effective and timely provision of hospice care for patients and their families outside of regular business hours, including evenings and weekends.
  • Assess patient needs over the phone until an in-person visit can be arranged.
  • Respond with urgency to crisis situations from patients and families during after-hours.
  • Conduct initial nursing evaluations to determine eligibility for hospice services within forty-eight (48) hours of referral when assigned.
  • Capable of performing after-hours visits, including admissions, death visits, recertifications, and routine assessments.
  • Identify and reassess the physical, psychosocial, emotional, and environmental needs of patients and families at least every fifteen (15) days.
  • Document issues, set appropriate goals, implement interventions, and record patient/family responses to care.

Qualifications

Experience Requirements
  • A minimum of one year of clinical experience.

Licensure Requirements
  • Active RN licensure in the state of practice and at least one year of clinical experience.
  • Current CPR Certification.
  • Valid driver's license, vehicle insurance, and access to reliable transportation or public transit.
