Comprehensive Physical Therapist

About the Job
This is a full-time position that requires 40 hours of work per week. The ideal candidate will be able to commit to an ongoing schedule with flexible shifts, including 8-hour days. As a member of our therapy team, you will be responsible for implementing physical therapy programs to prevent disability, restore function, and promote overall well-being for patients ranging from pediatric to geriatric age groups.
Key Responsibilities
* Deliver high-quality treatment programs to patients, ensuring compliance with established competency checklists and departmental guidelines.
* Conduct thorough assessments of patients' physical status, functional abilities, and age-specific needs to determine their current level of functioning and identify areas for improvement.
* Develop and implement individualized treatment plans that address patients' unique goals and objectives, involving them, their families, and social support systems in the decision-making process.
* Collaborate with other healthcare disciplines to coordinate therapy activities and advocate for patients' needs, facilitating environmental modifications and creating new support systems as necessary.
* Maintain accurate and timely documentation of patient progress, adhering to departmental and interdepartmental standards for record-keeping.
* Provide effective communication and education to patients, families, caregivers, and colleagues, utilizing written and verbal means as needed.
Requirements and Qualifications
* Graduate of an accredited program in physical therapy or equivalent experience.
* Recent graduates with certification in physical therapy are also considered.
* Current licensure as a Physical Therapist (Oregon) or Physical Therapy Permit (Oregon) within 6 months of hire, along with Basic Life Support certification within 2 months of hire.
* National Provider Identifier required upon hire.
