MDS Director - RN Leadership Position

Evergreen Center for Health & Rehabilitation seeks a skilled MDS Director to lead our team in delivering high-quality resident care plans.

Key Responsibilities:

  • Oversee the Minimum Data Set (MDS) assessments to ensure personalized care plans
  • Ensure compliance and accuracy in assessments
  • Monitor Case Mix Index (CMI) scores for reimbursement
  • Collaborate with interdisciplinary teams for accurate data collection
  • Provide ongoing education to facility staff and leaders
  • Track, trend, and analyze assessment data for compliance and optimization

Qualifications:

  • Valid state RN license preferred
  • Direct care and/or MDS Coordinator experience required
  • Proficient in state and federal regulations governing the MDS and billing process
  • Deadline driven, exceptionally detail-oriented individual
  • Excellent written and verbal communication and interpersonal abilities
