The Assessment Registered Nurse plays a vital role in the healthcare delivery system, working closely with the Care Management team to evaluate member eligibility for various programs offered by Centers Plan for Healthy Living.
Key Responsibilities:
- Conduct comprehensive assessments utilizing approved tools to determine member eligibility.
- Perform face-to-face evaluations in the member's residence, ensuring a thorough understanding of their needs.
- Identify and document medical, functional, and psychosocial requirements, engaging members and families in the care planning process.
- Collaborate with the interdisciplinary care team to develop tailored, person-centered care plans.
- Maintain compliance with all documentation standards and regulatory requirements.
- Participate in quality improvement initiatives and staff meetings to enhance service delivery.
Qualifications:
- Active and unrestricted RN license in New York State.
- Strong communication skills, both written and verbal.
- Ability to travel within the designated service area.
- Experience in home healthcare and managed care environments is preferred.
Physical Requirements:
Must be able to operate standard office equipment and communicate effectively in person and over the phone. Reasonable accommodations may be made for individuals with disabilities.
