Key Responsibilities:

  • Assess patients' physical, psychosocial, and environmental needs to develop and implement individualized care plans.
  • Provide skilled nursing care, including wound care, IV therapy, and medication management, as needed.
  • Collaborate with patients, families, and healthcare providers to ensure seamless care transitions and optimal patient outcomes.
  • Document patient progress, including assessments, interventions, and outcomes, in accordance with regulatory requirements.
  • Participate in interdisciplinary team meetings to discuss patient care and develop strategies for improvement.
  • Stay up-to-date with industry developments, best practices, and regulatory requirements to ensure high-quality patient care.

Requirements:

  • Current state license as a Registered Nurse.
  • 1-2 years of experience as a Registered Nurse in a clinical care setting.
  • 2+ years of Home Health experience.
  • Knowledge of OASIS and Homecare Homebase Software.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.

Preferred Qualifications:

  • CHPN certification.
  • Bachelor's degree in Nursing.
