Job Summary

A Social Services Coordinator is sought to support senior citizens in maintaining their independence and dignity within the community. The successful candidate will contribute to a caring and positive working and living environment for all employees and residents.

Key Responsibilities:

  • Build rapport with residents while maintaining professional boundaries to support their successful aging in place.
  • Identify and link residents to basic needs such as meals, housekeeping, transportation, personal services, financial assistance, counseling, and other social supports.
  • Support resident autonomy and dignity through the identification, linkage, and/or provision of services that promote safe, independent, and comfortable living.
  • Conduct assessments to identify resident needs and preferences, ensuring 95% of residents are contacted within the last 12 months.
  • Develop and facilitate resident activities, with a goal of attending 90% of activities per month.
  • Facilitate or arrange support groups to help residents process feelings and develop new relationships within the community.
  • Assist residents in acquiring In-Home Support Services where appropriate and available.
  • Help residents adhere to care plans and address behaviors that negatively impact community living, including conflict resolution between residents or resident and staff.
  • Advocate for individual residents within the parameters of HUD and other regulatory requirements, while maintaining resident privacy and rights.
  • Model and promote courteous, respectful, and non-confrontational communication skills.
  • Educate peers and others about the special needs of the resident population.

Qualifications and Requirements:

  • High school diploma or GED required.
  • Bachelor's degree in social work, psychology, or a related field strongly preferred.
  • Professional Service Coordinator (PSC) designation preferred.
  • Experience in an affordable housing environment preferred.
  • Demonstrated ability to serve with integrity, professionalism, and confidentiality.
  • Demonstrated working knowledge of supportive services and community resources.
  • Demonstrated ability to advocate, organize, problem-solve, and provide results for residents served.
  • Advanced interpersonal and communication skills.
  • Strong leadership skills with a proven ability to motivate others and deliver constructive feedback.
  • Strong analytical and problem-solving skills.
  • Ability to use mediation and conflict resolution techniques.
  • Excellent organizational skills, attention to detail, prioritization, and time management skills.
  • Proficient with Microsoft Office 365 and ability to quickly learn the organization's systems.

Reporting and Organizational Relationships:

This position will report directly to the Director of Resident Services and does not have supervisory responsibilities.

Compensation and Classification:

CCH is committed to paying competitive wages. The salary range for this position is $28.56 in California and $26.62 outside of California. This is a full-time, non-exempt position with a competitive benefits package.
